How do you take a picture with sunglasses that reflect the clouds or other scenery?

I have seen alot of photobucket pictures like that and I think it is amazing. I would like to know how to do it myself or if it is graphic work done in like Photoshop CS3 or something.
If I could get step by step instructions that would be great.
